TK-12 Registration 2024–2025

 

Below are all the instructions and resources needed to register and prepare for the new school year. Please check all three sections below, and complete steps 1–6 by Friday, Aug. 2.

Please confirm that your PowerSchool forms are completed.

PowerSchool registration opened in January. So, no further action is needed for those who completed this step early!

For those who have not yet completed the required annual registration forms, please log in to PowerSchool to complete those for each of your TK–12 children by Friday, Aug. 2. 

Instructions:

Option 1: PowerSchool Public Portal [desktop]:

  1. Log into your parent PowerSchool account using your username and password.
  2. Click Forms in the left navigation column to complete the 2024-2025 registration forms.

Option 2: PowerSchool Mobile App:

  1. Download the PowerSchool App on a mobile device and enter the following code to connect to Holy Family on PowerSchoolNZGG
  2. Touch More in the lower-right corner.
  3. Touch Forms to complete the 2024-2025 registration forms.

There are three sections of forms for review, and all forms must be completed in each section:

  • New Students Only - The Home Language Survey (HLS) form must only be completed once for new students.
  • Review & Update - These forms may have already been completed in past years but should be reviewed to ensure accurate information is on file with the schoolPlease note, they will be marked "Approved" for existing students and will update to "Pending Review" if changes are made. Please be sure to submit any changes to your address or contact information.
  • Required - These forms must be completed each school year.

Important:  

  • Contacts - It is critical to ensure accurate contact information for parental and emergency contacts. Please review the Student Contacts portion of this form to ensure you will receive school communications and can be contacted by the school in case of an emergency.

Tuition Payment Plans:

  • Tuition statements and payment schedules will be sent via email from FACTS by the end of summer.
  • For the 2024-2025 school year, returning families will be re-enrolled in their existing tuition payment plan. If you are a new TK-12 family, you will receive instructions on accessing your tuition account when statements are sent.

For those approved for an Education Savings Account (ESA):

  • If you have been approved for an ESA, please ensure you have selected your school in Odyssey.
  • Later this summer, Holy Family will verify your application and bill the first-semester tuition to your ESA. You will then log into your Odyssey account to confirm the tuition payment.
  • If you do not add your school or confirm your ESA payment in Odyssey, your ESA funds will not be applied to your tuition account.

All school and system communications, including delay and cancellation alerts, are delivered via ParentSquare.

  • If you are new to Holy Family, register your account at ParentSquare.com/signin using the cell phone number or email address you provided in PowerSchool (or look for an account activation email). Upon activation, you will have access to account settings, calendars, school directories, forms, conference sign-ups, volunteer opportunities and more!
  • All families are encouraged to log in to ParentSquare to review or adjust their notification settings for the new school year.

Please note that participation fees for secondary athletics and activities are invoiced separately. Wahlert Electronic Student Activity Passes will be sold through Bound (starting in August) for $95 per student.

Physical exam records are required for secondary students who participate in athletics. All students should provide up-to-date immunization records. Forms can be returned to your school at orientation or by email to bbroderick@holyfamilydbq.org.

PLEASE NOTE: All incoming seventh-graders must have the TDap and Meningococcal vaccines and 12th-graders must have the Meningococcal vaccine before school starts.

  • Visit the Meal Magic Family Portal, to view lunch balances, make payments, transfer funds between children, view meal purchase details and set account restrictions. 
  • If your family will need help paying for school meals this year, please fill out a Free & Reduced Meals application.
  • If your student requires a special diet, please fill out a Diet Modification form.
  • Did you know you can access the daily lunch menus on ParentSquare? Join the Elementary, Mazzuchelli and/or Wahlert Lunch group(s) to see the daily lunch menu in your ParentSquare calendar or subscribe to the calendar to sync it with your personal calendar.

Holy Family e-Store uniform and spirit apparel orders must be placed by July 22 to be delivered before school starts. 

Visit the Uniforms page for dress code information and to shop for uniform-approved items featuring Holy Family official logos. Items can be purchased through the Holy Family eStore or Lands’ End School. Crested (Holy Family branded) polos are required at the middle and high school levels, however, they are available for all schools.

Uniform samples (and Lands’ End Uniform Fit Kits) are available at the Holy Family Central Office at 2005 Kane St, Dubuque, Monday through Friday, 8 a.m. to 4 p.m.

Uniform Recycle Event Happening Now!

Our annual Uniform Recycle Event is taking place at Wahlert Catholic High School, 2005 Kane Street, from now through Friday, Aug. 2. Hours are Monday–Thursday, 8 a.m.–4 p.m., and Fridays, 8 a.m.–noon. Donate gently worn Holy Family uniforms and spirit wear. All items are free, with or without an apparel donation.

  • Visit the Holy Family e-Store to shop our 24-25 Booster Club spirit collection
  • Shop at the Booster Club Spirit Shop at Wahlert Catholic High School during office hours or during home fall athletics events.

Dubuque Community School District Busing

To qualify for busing, you must live more than two miles from your school (3 miles for high school students). Eligible families will receive a letter in August from the Dubuque Community School District indicating your bus route.

PLEASE NOTE: Holy Family students do not have busing on Friday mornings.

Transportation Reimbursement

If you qualify for busing but are not assigned a bus route, you may be eligible for transportation reimbursement. Submit the transportation reimbursement form before Dec. 1 for the full year. If you have a change in address, you will need to resubmit the form in the spring semester. Reimbursement payments are made in the fall of the following school year.

Jule City Transit

Families wishing to utilize The Jule for student transportation will need to purchase an Annual Youth Pass.

This year, Holy Family will be using Lifetouch for school pictures and memory books. Orders will be placed through the Lifetouch website. Stay tuned for ordering details from your school.

School Specific Information

  • Orientation for all 9th-grade students will take place on Wednesday, Aug. 21, from 8 a.m.–12 noon. Students should report to the Wahlert gym in dress code.  Orientation details for NEW 10–12-grade students will be sent at a later date. Please stay tuned for more details! 
  • Chromebooks will be distributed on Wednesday, Aug. 21, to all 9th–12th-grade students who are new to the Holy Family system.
  • Student Parking Passes can be purchased through ParentSquare.
  • School pictures for all students (except seniors) will be taken by Lifetouch during the school day on Monday, Sept. 10.
  • Incoming 12th-graders are required by the Iowa Department of Public Health to have a Meningococcal vaccine before they start school.
  • Incoming 9th-graders are required to submit a dental screening form.

  • An Ice Cream Social for incoming sixth-grade students and their families will be held at Mazzuchelli on Monday, Aug. 19, at 6–8 p.m.
  • Orientation for all Mazzuchelli students will be held on Thursday, Aug. 22. Sessions will be held at 8 a.m., 9:30 a.m., 4 p.m., and 5:30 p.m. Families may attend at the time most convenient for them – no sign-up is necessary. After an opening presentation in the gym at each session, students will have school pictures taken, lockers will be accessible, and school supplies can be delivered to mentors' classrooms. New-to-system 6th, 7th, and 8th graders can pick up their Chromebooks in the library.
  • Mazzuchelli spirit wear will be sold during orientation.
  • Class schedules will be provided during orientation and will be available for viewing on PowerSchool in mid-August.
  • School pictures will be taken by Lifetouch during orientation on Aug. 22. Purchasing pictures is optional, but all students must have pictures taken for school IDs. Dress code is not required. 
  • Immunization Requirements for incoming 7th-graders: Iowa Department of Public Health requires that all students entering the seventh grade have both a Meningococcal vaccine and a Tdap (Tetanus, Diptheria, and Pertussis/whooping cough) booster before school starts.   • Fall Sports Parent Meetings will be held on Monday, Aug. 26, at 4:40 p.m. for 7th-grade football; Tuesday, Aug. 27, at 4:15 for cross country; and 6 p.m. for 7th-grade volleyball. No preliminary sign-up is necessary to participate in sports. All participants must return the Athletic Physical form to the school office. View Mazzuchelli Athletics schedules here.
